We always advise that you contact your insurance prior to your visit to verify your personal coverage and benefit details. Most plans will require a copay or coinsurance after your deductible is paid, and there are often visit limitations for each calendar year; however, each patient’s plan is different. Additionally, some private insurance plans do not cover physical therapy as a payable benefit. Your insurance carrier can be reached by the phone number listed on the back of your insurance card.

** Our office is able in certain circumstances to initiate ‘one-time’ contracts for patients who are traveling outside of their home base area and require medical treatment, who may be unable to travel a greater distance to receive care at a contracted facility, etc. Our office is also able to treat patients injured due to automobile accidents. However, we are unable to bill for services rendered for injuries sustained when a ‘third party’ (someone other than yourself) is responsible – In these circumstances the patient will be required to pay our discounted cash rate, but will be given the necessary invoices to receive reimbursement from the third party payor.
